Book Launch homework day 6

Launch page for book https://sgboudreaux.com/launch-team-1 the link to apply does not work because I am unsure how to connect the application in google docs form to my website. I put the link on my page but it takes me to MY google docs account. When I preview the page it has automatically pasted my email address into the email section and I can't figure out how to make this not happen. Also, when copying the link to put into my website, it only copies the first section, not the entire document. Why would this happen?
