Have you done a beta-reader application?

I’d like to gather a few beta readers for my novel. The last time I did this, I had a wonderful response from eager readers who then did nothing. Their sight was farther than their reach, I’m sure, and I think several felt guilty. I don’t want them to get themselves in that position plus I’d really like some reader feedback. I did have two that gave me great response.

This time, I’m considering a short application form, clarifying that I’d like them to READ the novel and then answer a few questions. Have you done this? If so, could you share specifics on what you asked. Or what worked for you? Thank you!
