Novel Formatting Help

Hi! I'm currently working with a friend to self-publish an anthology, and as I was looking over their formatting plan, most of it seemed accurate. However, there were a few things that I was uncertain about and so I thought I would ask here!

First is that you don't indent your first paragraph in a chapter, right? Example:

Chapter- Chapter Title- First paragraph, not indented

Second, how do you properly format date and location? I always left-indented it…Example:

“A few days later…” or “Monday, Margaret's house”.

My friend has these formatted to be centered, even when it's a scene break in the middle of a chapter. Which is the correct way to format date and location? Centered, or left indented?
