Weird Vellum print issue

I got a proof copy from Amazon for a book I formatted on Vellum. Everything was fine except these beginning of the chapter pages. They squished it down and it only fills part of the page.

I had another copy printed from a local printer here in Uganda and they got it right.

Does anyone have any insight into what or why or how Amazon printed it like this? They are printing from the same PDF file.
